Bass Pro Shops offers Outfitters the choice of three plans – a traditional PPO plan and two high-deductible health (HDHP) plans that include a tax-advantaged Health Savings Account (HSA). All health plans are administered by Anthem Blue Cross and Blue Shield.

Options include: Choice PPO Plan, HDHP Core Plan, HDHP Value Plan

Health plans also include prescription coverage provided by CVS Caremark.

If you enroll in the Choice PPO Plan, you can elect to open a Health Care FSA. If you enroll in the HDHP Core or Value Plans, you can elect to open an HSA.

How Your Medical Plan Works

While our plans differ in how costs are shared, they work in much the same way.

Your Deductible

You pay 100 percent until your deductible is met.
Network preventive care is covered 100 percent and not subject to the deductible.
For the Choice PPO Plan, you pay a copay for some expenses in addition to your deductible, including doctor visits and prescriptions.

Your CoInsurance

Once you meet your deductible, the plan will begin to pay a share and you will pay a share of expenses.
You’ll always pay less when you see a provider in the network.
If you go out of the network, you’ll pay more.

Your Out-Of-Pocket Limit

This is the maximum you will pay each year for covered expenses. Once you reach your out-of-pocket limit, the plan will pay 100 percent of eligible expenses for the rest of the year.

Your Health Savings Account (HSA)

For HDHP Core and Value Plans, Bass Pro Shops will match your contributions dollar for dollar (100 percent) up to $500 for individual coverage or $1,000 for family coverage annually into your elected HSA.
