how to schedule an appointment using my Premise health

Appointments can be made by calling the Family Health Center or through the member portal, My Premise Health, at MyPremiseHealth.com.

  1. Log in to your My Premise Health account with your username and password. If you don’t have an account, you can create one using the “Sign Up Now” option.
  2. In the dashboard, select “Schedule an Appointment.”
  3. Select your desired appointment type from the available options.
  4. Choose your location (if applicable).
  5. Select a provider, date and time for your visit.
  6. Confirm appointment details. In the specified box, please provide any information you’d like your provider to know, such as questions or symptoms you may have. If this is your first time scheduling through the portal, you may be prompted to verify personal information before confirming appointment details.
  7. Your appointment is confirmed. Plan to arrive at your wellness center at your scheduled time. eCheck-In is not required for in-person appointments, but you may complete the process if you would like.

All Outfitters who work for Bass Pro Shops, and their dependent family members ages 2 and older who are covered on a Bass Pro Shops medical plan.

There is a flat office visit co-pay each time you see the physician:

  • For Outfitters covered under the Choice PPO medical plan, the office copay is $5.
  • For Outfitters covered under the Core or Value HSA plans, the office copay is $40.
  • For Outfitters not on Bass Pro Shops medical plan, the office copay is $75.

Many visits that do not require a doctor’s attention like lab draws, allergy shots, immunizations or wellness coaching and advice are paid entirely by Bass Pro Shops and offered free of charge.

Co-pays for the Bass Pro Shops Family Health Center are collected at the time of service. Payments can be made via cash, HSA or FSA debit card or credit card.

Yes, all Bass Pro Shops Outfitters and team members are welcome to schedule a visit and use the Health Center and Pharmacy.

For this reason, Bass Pro Shops has also enlisted Premise Health to provide Virtual Primary Care services to our nationwide Bass Pro Shops health plan members.

Prescription medications will follow the regular medical plan co-pays, deductibles and coinsurance. Those not on the medical plan will pay the discounted cost of the medication. We will not mark up the cost of prescription medications, so you are likely to find savings due to lower prescription ingredient costs. Over-the-counter products will be offered at discounted pricing.

You can request that the PCP share your records with the Bass Pro Shops Family Health Center by completing your provider’s record sharing process. Additionally, you may request your records from Premise Health be shared with your PCP or specialist.

No, the center is staffed and operated by Premise Health exclusively for Bass Pro Shops Outfitters.

Premise Health has recruited a high-quality team of dedicated healthcare professionals to staff and manage the Health Center. The team includes a family practice physician, an RN, LPN, and care manager, as well as a full-time pharmacist and pharmacy technician.

Yes. Premise Health will not share any personal health information without your permission. At no time is your personal health information shared with Bass Pro Shops. Your personal health information will remain confidential and your data will be kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records.

Yes. Bring in your prescription from your doctor or your authorized refill. New prescriptions can be filled. The pharmacy staff will assist with transferring from another pharmacy as needed.

Everyone’s time is best served if you can schedule an appointment. Scheduling is easy using the My Premise Health app or by calling. However, the Health Center will anticipate the needs for urgent care and do their best to see same-day appointments with minimal waiting as much as possible.

Yes. Premise Health will also provide Virtual Primary Care visits for all Bass Pro health plan members at the same low copays as the on-site health center. With Virtual Primary Care, you can see a Premise Health physician via video using the My Premise Health app.

With Virtual Primary Care, you can access a primary care provider nationwide for your annual wellness assessment, treat illnesses, refill medications and much more by phone or video. Also, unlike traditional telemedicine, virtual primary care allows you to designate a primary care provider (PCP) to see on an ongoing basis rather than speak with someone new every time you need care.
