Congratulations! New baby, a new adventure, new responsibilities! Bass Pro Shops offers extended family and medical leave, in accordance with applicable federal and state laws in effect at the time leave is granted. Leave may be taken to care for the Outfitter’s child after birth, or placement for adoption or foster care within 12 months following the birth or placement of the child.

benefit changes

If you would like to add your newborn/adopted child to your Bass Pro Shops Medical Plan, this action must be completed within 30 days of the event date (birth date or adoption date).

All changes to benefits are made in Workday. You will need to add the birth certificate or adoption documentation in Workday.

Log into Workday. Click on the Benefits icon > select Change Benefits > select Birth or Adoption

Apply for leave

Please complete all three steps to apply for FMLA or Bass Pro Medical Leave.

  1. Notify your supervisor of the need for leave.
  2. Call 1-888-973-3652 or visit https://www.TrackingAbsence.com/eep to begin your claim. Use the plan number on the Voya Leave Management card to the left.
  3. Sign any required documentation to release your medical information.
  4. Ensure your medical documentation is submitted to Voya, completed for approval, by due date given.

ready to return?

Complete the following steps to return from a leave of absence.

  • A return to work document or release note from your doctor must be emailed to your Voya Case Specialist or uploaded on the Voya app. 
  • Contact your site HR Representative or Supervisor of your date of return who will work with you directly on your schedule.
