As an Outfitter on the Bass Pro Shops Medical Plan, you have affordable, virtual access to connect with a doctor. We’ve partnered with Anthem and Premise Health to provide two convenient virtual health care options. Use these resources to receive your care on your time.
Chat with a doctor 24/7
The Sydney Health app by Anthem gives you the ability to receive virtual care over chat or video with a doctor anytime, anywhere. Anthem’s physicians are available and able to diagnose hundreds of conditions, follow up on previous care and coordinate labs for you. This simple care fits your lifestyle and doesn’t require you to make an appointment.

primary care from the comfort of home
New this year, you also have access to virtual primary care using My Premise Health. All U.S. Outfitters on our medical plan can use My Premise Health to schedule a virtual visit with one of Premise’s nationwide network of primary care providers include our own Dr. Sarah Duda, the Medical Director for the new Bass Pro Shops Family Health Center across the street from Base Camp.
Unlike traditional telemedicine, virtual primary care allows you to designate a primary care provider (PCP) to see on an ongoing basis rather than speak with someone new every time you need care.
To get started, visit MyPremiseHealth.com and follow these steps:
- Log in with your My Premise Health username and password, or “Sign Up Now”.
- Once logged in, click “Schedule an Appointment” in the dashboard.
- Select your desired appointment type from the available options listed. If you’re local to the Ozarks area, you can schedule an in-person visit at the Bass Pro Shops Family Health Center. For a virtual visit, you can either schedule a “New Member Visit-Video” or select “Get Care Now” to meet with the next available Premise Health team member on-demand by phone or video.
- Follow the rest of the steps as directed to confirm and schedule your appointment.
