You have accepted the job and are ready to enroll in benefits.

Your HR rep will guide you through this process. This is your initial offering of benefits, which means you may enroll in any plan offered to you without providing proof of good health. There are a few important things to note as you get started.

401K

ALL Outfitters may sign up and contribute to a 401k at any time. Contact Voya at 833-277-6401 or basspro401k.voya.com. You will receive a company match up to 4% of your contributions after one year of service.

EMPLOYEE ASSISTANCE PROGRAM

You are automatically enrolled in our Employee Assistance Program through Health Advocate. Learn about EAP offerings at healthadvocate.com/basspro or call 877-240-6863

VOLUNTARY INSURANCE

For full-time Outfitters, this is the best time to consider enrolling in extra life insurance through the Supplemental Group Life plans and/or Voluntary Short-Term and Long-Term Disability plans (for Hourly Outfitters). If you wait until the annual Open Enrollment, you must provide answers to Evidence of Insurability questions that will be evaluated by the plan carriers for approval.

Other voluntary benefit choices including Accident, Critical Illness and Hospital Indemnity plans.

YOUR BENEFITS

Once selected, your medical, dental and vision plans will continue until the end of the calendar year and may not be changed unless you experience a Qualifying Life Event.

A Health Savings Account (HSA) may be selected and can be changed or dropped at any time as long as you are enrolled in a High Deductible Health Plan (HDHP).
